Goodyear Orthodontic Treatments for the Prognathic Mandible
The two procedures used by the Goodyear orthodontist to shorten a prognathic mandible are the BSSO (bilateral sagittal split osteotomy) and the IVRO (internal vertical ramus osteotomy). The biggest advantage of the BSSO is that rigid internal fixation can be used. This means that maxillomandibular fixation is not necessary. In addition, there is a lower chance of relapse since rigid fixation is used. The final advantage of the BSSO is that more Goodyear surgeons are comfortable with this procedure, as they are not taught the IVRO in most residencies. The biggest disadvantage of the BSSO is the higher risk of permanent nerve disturbance. An additional disadvantage is that the surgery takes longer to complete than the IVRO.
The first advantage of the IVRO is that there is very little risk of permanent nerve disturbance. The next advantage is that there can be beneficial effects to the TMJ for patients with TMD signs or symptoms. The final advantage is that it requires less time to complete than the BSSO. The main disadvantage is that since rigid internal fixation cannot be used, maxillomandibular fixation for 6 weeks is necessary. This will require the patient to be on a completely liquid diet for 6 weeks. The other disadvantages are that few Goodyear surgeons are trained in this procedure and there is a greater chance of relapse.
