Goodyear Orthodontic Treatment for Asymmetric Mandibular Growth

If the condition has been rapidly progressing, the Goodyear orthodontist would expect to see a posterior open bite on the right side. As the right side of the mandible is growing quickly, it is unlikely that the teeth have had time to compensate leading to an open bite and crossbite on the right side. The maxillary occlusal plane would be relatively normal, but the mandibular occlusal place would be tipped down on the right. In addition, the patient’s chin would deviate to the left.

If the condition were slowly progressing, the posterior teeth would likely be in occlusion since they would have time to compensate. The maxillary and mandibular occlusal planes would be tipped down on the right. The mandibular incisors would probably tip right and the maxillary incisors would tip left. The chin would deviate to the left.

The Goodyear orthodontist would first take the standard records for a surgical patient: clinical exam, models, photos, panorex, lateral ceph, and PA ceph. In addition, the Goodyear orthodontist would send the patient for a technetium bone scan to determine if the condition is progressive and/or pathologic. Before beginning treatment, the Goodyear orthodontist would want to make sure growth has ceased. The Goodyear orthodontist would take serial cephalograms at 1-year intervals until growth has ceased. The goal of the presurgical orthodontics will be to decompensate the arches. The surgical plan will depend upon if the maxilla has compensated for the mandibular growth. If the maxilla does not have a cant, the Goodyear orthodontist would recommend an IVRO with an asymmetric setback. If the maxilla does have a cant, the Goodyear orthodontist would recommend a LeFort I osteotomy to correct the maxillary cant as well as an IVRO with an asymmetric setback.
